Chapter 6: About Pancreatic Tumors

      Professor Xu just introduced the effects of combined immunotherapy for gastric cancer and liver cancer. Almost every case has a magical story. His overall survival from receiving complex immunotherapy was 3 years and 3 months. I remember when he was critically ill, I flew to his home in Kuala Lumpur to visit him. As soon as I entered his house, my tears flowed down. His father immediately grabbed me and said, "Professor, don't be sad. Our Weitian has completed what he should have done and will soon return to God." " This reminds me of December 21, 2011, when I was invited by Wei Tian to attend the "Christmas Concert" conducted by him at the Kuala Lumpur Cultural Center. Before the meeting, his father and I drank coffee in the cafe. His father looked at me deeply and said, "Professor Xu, I am his biological father, and you are his adopted father.       A doctor should have a caring heart. Professor Xu has a caring heart. The value of life lies not only in its length, but also in its width. Chen Weitian should be lucky. Pavarotti, a famous Italian tenor singer who is far more famous than him, was diagnosed with pancreatic cancer in 2006 and underwent tumor removal surgery in the United States. Less than a year later, he passed away on September 6, 2007.

      On the same day as Chen Weitian in May 2010, another pancreatic tumor patient named Liangliang came to consult me in Malaysia. His family said that he was "comatose" every morning. CT scans showed that there were 3 tumors in the body and tail of the pancreas. In April 2011, Liangliang's condition worsened and he was sent to our hospital on June 8 under the escort of his family. At that time, he was short of breath and his blood sugar was only 1.4 mmol/L. There was a large amount of effusion in the pericardium and thoracic cavity, multiple metastatic lesions in the bones, adrenal glands and liver, and a pancreatic biopsy confirmed a malignant neuroendocrine tumor (NEC). Fasting blood insulin was 8 times higher than normal, indicating a functional tumor. Liangliang received percutaneous cryoablation of the pancreatic tumor. Blood sugar quickly dropped to normal after the operation. Starting from June 20, 2011, Liangliang received combined immunotherapy for more than 3 years. Liangliang almost completely recovered to normal and got married. Until June 2017, he had a severe lung infection in Malaysia and died soon after. Since entering Fudan University Cancer Hospital, Liangliang has been "progression-free" for a full 6 years.

      Perhaps people are not unfamiliar with this tumor, because "Apple" Steve Jobs suffered from this tumor. In October 2003, Jobs underwent Whipple surgery to remove most of his pancreas and underwent a liver transplant. 2011 In October, this great man who made great contributions to the world passed away at the age of 56. From diagnosis to death, Jobs survived for 8 years. During these 8 years, Jobs received the world's "most advanced" modern treatments, but despite this, his life was not saved. In addition, these treatments brought him a lot of "confusion". The patient treated by Professor Xu was the most ordinary person and it was impossible for him to receive many "modern" treatments. He survived by only receiving simple treatments such as freezing (local) and compound immunity (system). He lived for more than 6 years and enjoyed life. In the end, although he passed away, he did not die of cancer, but of infection. It is true: the way is simple, and simplicity brings out the magic.

      We also treated a 17-year-old beautiful girl named Xiaofeng who had been "abandoned" in a more "simple" way. It was a Sunday in June 2014, and I went to the hospital for inspection in the morning. The attending physician of the ward told me that a girl was discharged after being hospitalized for 2 days. I asked what disease she had. The answer was "a strange pancreatic disease, very rare". I asked where the patient was now? The nurse next to her said "I went home, just left this morning", "Where is home?" The nurse said: In Huaiji, 300 kilometers away from Guangzhou. I called the driver, had lunch, and led the attending physician to Huaiji. I have been there before. Mingzai, a patient we treated many years ago, lived there. It was very poor and in the mountains. Five hours later, we arrived at Xiaofeng's home. Her condition was indeed serious. In March 2009, she underwent surgery for a large space-occupying lesion in the left upper and middle abdomen. Postoperative pathology showed pancreatic solid-pseudopapillary tumor. No further treatment was received. In recent years, Xiaofeng has been working in Shenzhen and has recently become a dance instructor. Until January 2014, Xiaofeng developed abdominal pain, lower limb edema, and weight loss. CT showed multiple nodular and mass lesions in the liver, occupying 40% of the whole liver, portal vein cancer thrombus formation, multiple lymph node enlargement, and pathology showed that liver metastasis came from pancreatic parenchyma-pseudopapillary tumor recurrence. After much persuasion, we finally persuaded Xiaofeng to return to the hospital. However, a few days after admission, Xiaofeng's condition deteriorated sharply, with jaundice and ascites, a significant decrease in platelets, and a 50% increase in prothrombin time, presenting with DIC, a fatal blood coagulation complication. After actively giving symptomatic treatment, we injected her with compound immune agents from August 25, 2014. Three weeks later, Xiaofeng's jaundice and ascites completely subsided, and she was discharged from the hospital to rest. After that, the compound immune agent was injected subcutaneously once a week for 15 months. Xiaofeng has been working normally in Shenzhen so far, and has recently become a dance instructor. Multiple CT follow-up showed that the huge metastatic tumor in the liver still exists and has not progressed significantly.

      The liver metastasis in this case has not changed significantly since 2016, and the combined immunotherapy has only been done for 15 months, but the patient is still alive and well. This miraculous case at least shows the following points: (1) "Coexisting with cancer" is feasible; (2) The balance between immunity and cancer is enough to promote cancer "dormancy"; (3) The combined immunotherapy agent can not only take effect quickly, but also has a lasting effect.
